I hope this helps you . you have 4 elements set to width:50% which equals 200% width. Change them to width: 24% (inline-block elements have a natural spacing of 1 or 2px) or less and they will be aligned:
code :
#form input[name="1"] {
  margin-top: 8px;
  height: 44px;
  width: 24%; <---------------
  background: #A901DB;
  border-bottom: 1px solid #B404AE;
  color: #FFF;
  font-size: 18px;
  text-align: center;
}

Placing contents of form and map side by side with HTML5

Placing contents of form and map side by side with HTML5

seems to work fine I want to put the contents of map and form side by side. I want the code to have responsive design. I have tried putting contents in table and list format. But still form is on top of map. I want to have contents side by side when it is displayed on a wider screen. While on smaller width screens have them on one over the other. , Well, that's pretty simple with medias queries ...
HTML
code :
<div id="map"> <!-- your map --> </div>
<div id="form"> <!-- your form --> </div>
#map, #form {
    width: 50%;
    float: left;
}

@media handheld, only screen and (max-width: 500px), only screen and (max-device-width: 500px) {
    #map, #form {
        width: auto;
        float: none;
    }
}

it helps some times I couldn't seem to put both of the headers next to each other in the same line. And also, I would like to align the headers to the center of the page.
code :
.inline-div {
  display: block;
  text-align: center;
}

h4
{
  display: inline-block;
  text-align: center;
}
<div class="inline-div">
  <h4>Product Tested :</h4>
  <h4>Profile Tested :</h4>
</div>
