hope this fix your issue Serial data is just a stream of data. Depending on when you read it and the buffering that is happening, only part of the data may be available when you read it.
Since you are using line oriented data, what you will want to do is buffer the data until you see the line terminator and only then process the data.

Does that help One thing you could try to confirm that it's nothing with the RXTX library, or your cable/adapter, you could open up a hyperterminal session at each computer, configure equal serial port parameters, and start typing in one of them - the text should appear in the other, and vice versa. If you have that working, then it's just a problem with the code.
I found this good example. You could take out the writing part and put that at one computer, and the reading part and put it in the other.

I hope this helps . Based on the provided code the only suggestion I could give is to try flushing the stream (out.flush()) after putting data in it.
Other than that - I'd need to see the code that gets the output stream.

I wish this helpful for you I believe their official site is down. I recently downloaded java RxTx here:
http://java2s.com/Code/Jar/r/Downloadrxtx22jar.htm - jar file containing RxTx library

I wish this help you Note: I fix the Problem, I rewrite the serialreader class in this way and everything work fine now:
code :
public static class SerialReader implements Runnable
    {
        InputStream in;
        CommPort serialport;

        public SerialReader (InputStream in, CommPort port)
        {
            this.in = in;
            serialport = port;

        }

        public void run ()
        {
            byte[] buffer = new byte[1024];
            int len = -1;
            try
            {
                String txt = "";
                long startTime = System.currentTimeMillis(); //fetch starting time
                while(( len = this.in.read(buffer)) > -1 & (System.currentTimeMillis()-startTime)<6000)
                {
                    System.out.print(new String(buffer,0,len));
                    txt = txt + (new String(buffer,0,len));
                }
              PrintWriter writer = new PrintWriter("Patient" + startTime + ".txt", "UTF-8");
              writer.println(txt);
              writer.close();
              this.in.close();
              serialport.close();

            }
            catch ( IOException e )
            {
                e.printStackTrace();
            }

        }

    }
