I wish did fix the issue. Since your code is not in a function hence return false; is not working as you expect, and code continues to execute below. Your comparisons may already be fine. Change that return false; to die();
Ok so here is probably where the problem lies
code :
if($firstname=="" || $lastname=="" || $email=="" || $pass=="" || $conditions==""){
if(isset($_POST["firstname"]) && isset($_POST["lastname"]))
 // all your php code

it helps some times Assuming the code was working and only the validation doesn't - my guess is that you wanted something like the following:
code :
onsubmit="return inputValidation()"
<SCRIPT LANGUAGE="JavaScript" type="text/javascript">
function inputValidation()
  var dateBox= document.forms["form"]["date"].value;
  if (dateBox== null || dateBox== "")
      alert("Please enter an approximate date.");
      return false;
  return true;
<form name="form" id="form" action="add.php" onsubmit="return inputValidation()" method="post">
  <fieldset id="contactFields">
  <legend>Client Information</legend><br />
<p id="require"><span>*</span>Required information</p><br />
<label class="blockLabel">Approximate Date Serviced: <span>*</span><br/><input type="date" name = "date" id="date"/><br/><br/>
<label class="blockLabel">First Name: <span>*</span><br/><input type="text" name = "firstName" id="firstName"/><br/><br/>
<label class="blockLabel">Last Name: <span>*</span><br/><input type="text" name = "lastName" id="lastName"/><br/><br/>

<label class="blockLabel">Comments:<span>*</span><br/><textarea name = "comment" id="comment" rows="5" cols="55"></textarea>

<label id="wordcountLabel" for="wordcount">
<div>Character Count:<input type="text" id="wordcount" readonly="readonly" value="0/0" /></div>

  <div id="submit"><input text-align="center" class="button1" type="submit" value="Submit" /></div>

<input text-align="center" class="button3" type="reset" value="Reset" />

I wish this helpful for you After you echo out the error you need to remove the variable from the session.
Add unset($_SESSION['result']); under:
code :
    echo "<font color=white>Invalid Credentials. Try again!!! </font>";
    echo "<font color=white>User doesn't exist. Try again!!!</font>";

I wish this help you Your validator will fail on the email, because you are feeding a string to .match, when it needs a regex.
You also have to call .match on the email itself, with the regex as the argument.
code :
var reStr = "^[A-Z0-9._%+-]+@[A-Z0-9.-]+\.[A-Z]{2,}$";
var re = /^[A-Z0-9._%+-]+@[A-Z0-9.-]+\.[A-Z]{2,}$/i;
var re = new RegExp("^[A-Z0-9._%+-]+@[A-Z0-9.-]+\.[A-Z]{2,}$", "i");
var re = /^[A-Z0-9._%+-]+@[A-Z0-9.-]+\.[A-Z]{2,}$/i;
