fixed the issue. Will look into that further I want to put this: <%= f.text_field :name %> , You dont' need the whole input section, just do this :
code :
<%= f.text_field :name , :class=> "form-control"%>

Any of those help You have the right idea, you just have to put everything that is either to be appended/prepended or have something appended/prepended to it on one line with no spaces.
Like this:
code :
<form class="form-inline">
  <div class="control-group">
    <div class="controls">
      <div class="input-prepend input-append">
        <span class="add-on">Search</span><input id="search" class="input-xlarge" type="text" size="16"><button class="btn" type="button">Go!</button>

this will help A simple way to put a basic form up as a web site is Sinatra. The following is a web app in a single file, using Sinatra.
code :
#!/usr/bin/env ruby
require 'sinatra'

get '/' do
  erb :guess

post '/' do
  @guess = params[:guess].to_i
  if @guess == 10
    @message = "Correct!"
    @message = "Try again..."
  erb :guess

@@ layout
   <%= yield %>

@@ guess
<form action="" method="post">
  <p>Guess a number: <input type="text" name="guess"/></p>
  <p><%= @message %></p>

hop of those help? I understand that you're referencing to bootstrap's default stylesheet, but there are a couple of things you need to learn about first. In order to manipulate the position or appearance of anything in bootstrap according to your own preferences you must make a custom stylesheet and then make sure that it's referenced to after your bootstrap stylesheets so that bootstrap doesn't negate your styles. You then must know enough css in order to change your styles according to your preferences. You're currently using javascript to place a background image in a div which is not semantically correct. Here's a link explaining basics, but you'll really need to learn more about css in order to accomplish what you want with bootstrap. http://bootstrapbay.com/blog/customize-bootstrap/

wish helps you I'm wanting to create a responsive Mailchimp embed form with Bootstrap. I embedded the form but it's not responsive. It's only an issue for mobile devices. I want to be able to make the submit bottom go below the email input for mobile devices and both of these fields to be centred. , Just add this to your css Elliott
code :
@media (max-width:768px){
    #mc_embed_signup .clear {clear:none; display:block !important;}

this one helps. I actually figured out a proper solution without having to do any redirects or hairy coding.
First I setup a local host, which was my biggest issue, this helps with getting the actual hubspot form to show within my page (using a local show the embedd code does not appear, although obvious to others, it took me a sec). Oh, I should mention that the code is not necessary.
